Zorgdekking in Spanje: EHIC, S1, A1 of privé?

Zorgdekking is niet één document. Het is een keten die je werk- of woonsituatie verbindt met het land dat voor jou verantwoordelijk is, het bewijs daarvan, en het zorgstelsel dat je daadwerkelijk kunt gebruiken.

Het korte antwoord

Two separate questions decide everything: who pays for your healthcare, and how you actually access it in Spain. They are not always answered by the same country or the same document.

If you work and contribute in Spain, Spanish Social Security normally covers you. If another European country stays responsible, an S1 registers that cover here. An EHIC covers temporary stays. An A1 only says where you pay contributions. Private insurance fills the gap when no public system covers you.

Twee vragen, niet één

When you move to Spain, you need to answer two separate questions. Who is responsible for paying for your healthcare, and how do you actually access healthcare in Spain. These are not always the same thing.

You may be covered by Spanish Social Security because you work in Spain. You may remain insured in another European country and register that coverage in Spain through an S1 form. You may only have temporary cover through an EHIC. Or you may need private health insurance.

The correct option depends mainly on whether you are working, studying, retired, temporarily posted to Spain or living here without employment.

Publieke en private zorg in Spanje

Spain has a public healthcare system called the Sistema Nacional de Salud. It is funded mainly through public funds and organised by the autonomous communities, and it includes primary care, specialist care, hospital treatment and emergency treatment.

Once your entitlement has been recognised, you normally register with the healthcare service of your autonomous community and receive a regional health card, the tarjeta sanitaria individual. The system assigns you a health centre and usually a family doctor according to your registered address, and non-emergency specialist care normally starts with a referral from that doctor.

Private healthcare operates separately. You normally choose doctors, clinics and hospitals from the insurer's medical network, appointments may be available more quickly, and direct access to specialists is often easier depending on the policy. Having private insurance does not automatically cancel public healthcare rights, and some people hold both.

A private policy may satisfy a residence requirement, but it does not register you in the Spanish public health system. In the same way, having a Spanish Social Security number does not necessarily mean your public healthcare registration has already been completed.

When you work as an employee or as a registered self-employed person in Spain, Spain will normally become the country responsible for your Social Security and healthcare. Your employer registers you with the system, or you register yourself when becoming self-employed.

These are connected steps, but they are not the same document.

Een NUSS geeft je niet automatisch een zorgpas

An NUSS is only the identification number used by Social Security, and you may receive it before your employment starts. Public healthcare normally depends on your actual entitlement, such as being registered as an employee, self-employed person, pensioner or beneficiary. You may still need to obtain the entitlement certificate and register with the regional health authority before receiving your card.

Wat er gebeurt als je stopt met werken

Losing a job does not always mean that public healthcare stops immediately. Your position may continue through unemployment benefits, continued entitlement, residence-based rules or another recognised status. But the correct basis must be checked: an old Social Security number or an expired employment relationship does not provide healthcare indefinitely.

De Europese zorgpas

The EHIC proves that you are insured under the public healthcare system of another participating country. It is designed for temporary stays, and it allows you to receive medically necessary treatment from the Spanish public system under the same conditions and at the same cost as people insured in Spain.

Medically necessary does not mean emergencies only. It can include treatment that cannot reasonably wait until you return home, taking into account your medical condition and the expected duration of your stay: treatment connected to an accident, sudden illness, an existing condition, pregnancy or other necessary care. The medical provider decides what is necessary in the circumstances.

Wat de EHIC niet dekt

  • ×Private healthcare. You must use providers operating within the Spanish public system, and make clear you are seeking treatment under the EHIC.
  • ×Repatriation, rescue costs or lost belongings. It is not a substitute for travel insurance.
  • ×Travel for planned treatment. Separate authorisation rules apply to planned treatment abroad.
  • ×A guarantee that everything is free. You are treated under the same rules as locally insured patients, so charges that apply within the Spanish public system also apply to you.

Is de EHIC genoeg als je naar Spanje verhuist?

Usually not as a permanent solution. The EHIC is intended for temporary stays, while someone transferring their habitual residence to Spain normally needs a more permanent basis: Spanish Social Security through work, an S1 because another country remains responsible, or private insurance.

Eén uitzondering, voor sommige studenten

For an EU student applying for the CUE, a valid EHIC may in practice satisfy the health-cover requirement when it covers the full expected residence period and gives access to medically necessary care. That does not turn it into full permanent Spanish public health registration; it means it may be accepted as sufficient evidence for the residence application. A worker relocating permanently should not normally rely on an EHIC once Spanish Social Security becomes applicable.

Het S1-formulier

The S1 is one of the most important and misunderstood documents for people who live in Spain but remain insured in another European country. Officially a Portable Document S1, older versions were known as forms E106, E109, E120 or E121.

It certifies that one country remains financially responsible for your healthcare even though you live in another. You request it from the healthcare or Social Security institution of the country responsible for your cover, then register it in Spain. Once registered, you access the Spanish public system under the same general conditions as people insured here, while the cost remains chargeable to the issuing country.

The S1 does not mean that you hold private insurance. It connects two public Social Security or healthcare systems.

Pensioners whose pension-paying country remains responsible for their healthcare, workers posted temporarily to another country, certain cross-border workers, civil servants and eligible dependent family members. It may also apply in other cross-border situations where EU coordination rules place the country of insurance outside the country of residence.

You cannot simply choose to obtain an S1 because you prefer to remain covered in your home country. The competent institution must first determine that its country remains responsible.

Wordt een S1 geaccepteerd voor het CUE?

Yes, when it proves valid public healthcare coverage in Spain for the relevant period. Spanish rules allow health insurance to be public or private, and contracted in Spain or another country, provided it gives coverage in Spain equivalent to that provided by the Spanish National Health System. A properly issued and registered S1 is generally strong evidence, and for pensioners the rules expressly recognise certification showing that healthcare is covered by the state paying the pension.

Het A1-formulier

The A1 is often mentioned together with the S1, but it serves a different purpose. It does not directly register you for healthcare. It certifies which country's Social Security legislation applies to you while you work across borders.

Under the normal rule, a person is subject to the Social Security system of the country where they work. EU coordination rules contain exceptions for temporary postings, work in several countries and certain other cross-border arrangements. An A1 confirms that, despite working temporarily or partly in Spain, you remain subject to another country's system, and that Spanish contributions should not be required for the work it covers.

How do I access healthcare where I live when another country is responsible?

A person may need both. An employee posted by a Dutch company to work temporarily in Spain may hold a Dutch A1 confirming that Dutch Social Security continues to apply. If the stay is genuinely temporary and their habitual residence does not move, an EHIC may be enough for medically necessary public healthcare. If they do move their residence to Spain during the posting, they may also need an S1 to register for full public access.

The A1 alone should therefore not be described as health insurance. It helps establish which country is responsible, but the EHIC or S1 is normally the document used to access healthcare.

For a posted employee, the employer normally requests it from the Social Security institution in the country where the employee is insured. For a self-employed person or someone working in several countries, the application may be made by the individual or handled according to the procedure of the competent national institution. A posted worker can generally remain covered by the home-country system for a defined period where the legal conditions are met; confirm the exact duration with the relevant home-country social security institution, since longer postings require a different assessment or an agreement between the institutions.

Op afstand werken vanuit Spanje is niet automatisch detachering

Continuing to work online for a foreign employer after moving to Spain does not automatically mean you remain insured in the employer's country. The applicable country depends on the actual working arrangement, where the work is performed, whether the move is temporary, whether the employer formally posted you, and whether you work in more than one country.

When a person normally works in more than one EU country, the country of residence may become responsible if a substantial part of the work is performed there. EU guidance normally uses a working-time or remuneration threshold as the indicator, although the complete rules must be applied to the individual situation. The employer should obtain a formal determination and A1 where appropriate rather than assuming contributions can continue abroad indefinitely.

Private zorgverzekering

Private insurance is normally relevant when you do not yet qualify for Spanish public healthcare and no other European country is responsible for your cover. It may also be taken out voluntarily by someone who already has public healthcare but wants faster access, more provider choice or additional services.

For immigration and CUE purposes, the policy must do more than provide emergency or travel cover. Spanish rules require non-working EU residents to show public or private health insurance, contracted in Spain or another country, that provides coverage in Spain during the residence period equivalent to the coverage provided by the Spanish National Health System.

For a CUE application, the safest policy is comprehensive medical insurance valid throughout Spain, without major exclusions that leave ordinary healthcare uncovered. In practice, immigration offices commonly expect full medical and hospital coverage and no patient co-payments.

The legislation does not reduce the requirement to a particular marketing label. What matters legally is the actual level of cover in Spain. But offices often assess policies through the insurer's certificate, so that document should clearly confirm the insured person, period of validity, territorial coverage and nature of the cover.

A policy purchased from an insurer outside Spain can legally qualify if it provides equivalent coverage in Spain. In practice a foreign policy can be harder to prove: the authority may ask for translated terms, detailed coverage confirmation, and evidence that treatment in Spain is directly available rather than only reimbursed after payment.

Bestaande aandoeningen en wachttijden

Policies can exclude illnesses that existed before the contract began, and may impose waiting periods before hospitalisation, surgery, maternity care or certain treatments are covered. These restrictions matter both medically and administratively: a policy chosen only because it is inexpensive may not provide the expected treatment, or may not be accepted as sufficiently comprehensive. Check the full policy wording, not only the certificate.

Geeft een private verzekering je een Spaanse publieke zorgpas?

No. Private insurance gives access to the insurer's private network or reimbursement system. It does not register you with the Spanish public health service. Emergency public treatment may still be provided where medically necessary, but that does not mean you have general public healthcare entitlement.

Publiek, privaat of allebei?

Public healthcare in Spain is broad and organised geographically, delivered through public health centres and hospitals rather than an insurer's list of private doctors. Private insurance is contractual, and your rights depend on the policy, network, exclusions, waiting periods and payment conditions.

Public healthcare is usually the natural primary cover for people working and contributing in Spain, or whose S1 has been registered here. Private insurance is normally the primary solution for non-working residents who cannot rely on an S1 or Spanish Social Security, and can remain useful as supplementary cover afterwards. Paying Spanish Social Security contributions does not purchase a private policy, and paying private premiums does not count as Spanish Social Security registration.

Welke zorgdekking heb je nodig voor het CUE?

The health-cover requirement depends on the category under which you apply.

You work in Spain

An employee or registered self-employed person normally demonstrates status through employment or Spanish Social Security registration. Separate private insurance is generally not required, because the worker's status provides access to public healthcare. Show that the activity is genuine and that the corresponding registration has been completed or can be verified.

You are a student

You must generally show cover during the residence period. A valid EHIC can in practice be accepted where its validity covers the entire intended stay and it provides access to medically necessary healthcare. Without a suitable EHIC, an S1 or comprehensive private insurance may be used. Do not assume a digital national insurance card proves international coverage; the document must demonstrate entitlement in Spain.

You do not work in Spain

You normally need comprehensive health insurance together with sufficient financial resources. This may be Spanish private insurance, qualifying foreign insurance, or an S1 showing that another public system remains responsible. An EHIC is usually not the correct long-term solution once you have transferred your habitual residence.

You are a pensioner

The country responsible for your pension will often also be responsible for your healthcare. When another European country remains responsible, request an S1 from it and register it in Spain, giving you access to public healthcare while the issuing country bears the cost. If Spain pays the pension, or Spanish rules otherwise make Spain responsible, healthcare may be recognised directly through the Spanish system.

You are temporarily posted to Spain

Your employer should determine whether you remain under the sending country's system and obtain an A1. For healthcare, an EHIC may cover a genuinely temporary stay; when you relocate your residence during the posting, an S1 may be required to register with the Spanish public system. The A1 confirms the applicable legislation, it does not replace the EHIC or S1.

You work remotely for a foreign company

This must be assessed rather than guessed. You may become subject to Spanish Social Security because the work is physically performed from Spain, or the foreign system may remain applicable under posting or multi-state-worker rules and issue an A1. Where the foreign country remains responsible and you reside in Spain, an S1 may then be required. Private insurance may cover you medically in the meantime, but it does not resolve where contributions legally belong.

Als je nu in een ander EU-land verzekerd bent

Do not begin by asking whether your existing insurance card works in Spain. First determine whether your stay is temporary, or whether Spain is becoming your country of residence.

A temporary stay. Request an EHIC from your current national health insurer and verify its expiry date.

Moving here while another country stays responsible. Request an S1 and register it with the INSS after arrival.

Moving to work for a Spanish employer. Spanish Social Security will normally take over. Your previous EHIC should not be treated as permanent coverage once the applicable system changes.

Posted temporarily by a foreign employer. The employer should request an A1. You then use an EHIC for a temporary stay, or an S1 when you reside in Spain.

No public system will remain responsible. Arrange comprehensive private insurance valid in Spain.

The institution in the country where you are currently insured must confirm whether it can issue an A1 or S1. Neither document is issued merely because you request one; eligibility follows the European coordination rules.

Veelvoorkomende misverstanden

"I have an EHIC, so I am fully insured after moving to Spain."

Not necessarily. The EHIC is intended for medically necessary care during a temporary stay. It does not normally replace Spanish Social Security, an S1 or comprehensive private insurance after you transfer your residence to Spain.

"I have an A1, so I can register with a Spanish doctor."

Not on the basis of the A1 alone. It confirms which Social Security system applies to your work. You normally need an EHIC or S1 to establish how healthcare is accessed in Spain.

"An S1 is private insurance."

No. An S1 coordinates public healthcare between two countries. You receive treatment through the Spanish public system, while another state remains financially responsible.

"I have an NUSS, so I already have public healthcare."

Not necessarily. The NUSS is your Social Security identification number. Your healthcare entitlement depends on your recognised status, such as being registered as working, being a beneficiary, or having an S1 registered.

"Any health insurance is enough for the CUE."

No. A travel policy or emergency-only plan may not provide the level of coverage required. For non-working applicants, the cover must be valid in Spain and equivalent to the Spanish National Health System for the residence period.

"Once I obtain private insurance, I cannot use public healthcare later."

Incorrect. You can move from private cover to public entitlement when your circumstances change, such as when you start working in Spain or register an S1. You may also keep both types of cover.
